Aging and Disability Resource Center

This initiative is to provide a coordinated system of information and access through a single point of entry to all publicly funded long-term supports, including those funded by Medicaid, the Older Americans Act, and other state and federal programs and services. This is done by assisting seniors and people with disabilities, their families and caregivers by helping them navigate the maze of social services and health care empowering them to make informed choices via website, phone, or at the Area Agency on Aging office.

What is the Aging and Disability Resource Center?

The Aging and Disability Resource Center (ADRC) is a single point of contact for information about publicly funded long-term services and support. NACOLG’s ADRC helps older adults, people with disabilities, family members and caregivers understand available programs and make informed decisions about care.

Can the ADRC help me find services for an aging parent?

Yes. Family members may contact the ADRC for help finding services and support for an older parent or loved one. An ADRC specialist can listen to your concerns, explain possible options and direct you to the appropriate programs or agencies.

Can caregivers contact the ADRC?

Yes. Caregivers may contact the ADRC for information, referrals and help navigating aging and disability services. You do not have to wait until a crisis occurs to ask for assistance.

Can the ADRC help someone with a disability?

Yes. The ADRC assists people with disabilities, as well as their families and caregivers, in identifying available services and long-term support options.

Does the ADRC determine whether I qualify for Medicaid?

The ADRC can provide information about Medicaid-funded long-term services and direct you to the appropriate application or eligibility resources. Final eligibility decisions are made by the agency responsible for administering the applicable program.

Can the ADRC help me remain independent at home?

The ADRC can help identify programs and community resources that may support safety, independence and quality of life at home. Available services depend on a person’s needs, location and program eligibility.

Do I need to know which program I want before calling?

No. One purpose of the ADRC is to simplify the process of finding help. Explain your situation to an ADRC specialist, and they can help identify services or programs that may meet your needs.

Can I receive assistance without visiting the NACOLG office?

Yes. Information and assistance are available by phone and through the NACOLG website. You may also visit the Area Agency on Aging office if you prefer in-person assistance.

No. Anyone seeking information about aging, disability or long-term support services may contact the ADRC. An ADRC specialist can explain available options and help you determine which programs may be appropriate.
